Change is important to improve patient safety and quality of care, especially reducing associated infections. Change models guide the implementation of innovative and quality improvement (QI) initiatives within the nursing practice and healthcare organizations. The proposed EBP project focuses on providing training for nurses to enhance their knowledge on best practices in using catheters; especially their care and insertion with a view of reducing rates of CAUTIs among hospitalized patients with indwelling urinary catheters (Abdelmoaty et al., 2022). The purpose of this paper is to present the EBP framework or model for change that will be used in implementing the proposed intervention.

Selected Model or Framework & Relevance to the Project

The selected change model is the John Hopkins evidence-based practice approach. The framework aims at addressing clinical problems and using the most appropriate interventions in a user-friendly manner to guide changes in nursing and healthcare settings (Dang et al., 2021). The model aligns with the proposed EBP project whose aim is to implement training for nurses on the best practices in catheterization, especially through the use of Foley catheters to reduce CAUTIs. Being a problem-solving model, the John Hopkins framework is relevant to the EBP project as it will facilitate change by addressing the clinical issue of limited or lack of awareness about best practices in catheterization to reduce CAUTIs.

Stages of the John Hopkins EBP Model

Evidence-based practices are important in making clinical decisions and processes based on a problem-solving approach that focuses on change. EBP allows nurses to incorporate proven scientific evidence to improve quality of care. Consequently, the John Hopkins EBP model follows three critical steps during change initiative’s implementation (Watters, 2019). These include practice question, evidence, and transition. The initial step is to develop and refine the implementation question and the team that will establish the change. The stage entails identification of the existing health issue, determining change’s purpose, and assembling tools and a team to facilitate the change.

The second phase is the evidence step and entails searching for, appraising, and synthesizing internal and external sources for evidence. Based on this stage, one develops a PICOT question and change managers identify the most reputable and reliable sources of research like books, medical website, and research journals. The team then appraises the evidence to ascertain that it answers the aspects of the PICOT question. The third stage is the transition which entails creating and implementing an action plan, evaluating the outcomes, and disseminating the findings to the appropriate individuals in the facility (Dang et al., 2021). The third stage entails implementing changes through enabling team members to undertake certain roles to facilitate the transformation. The John Hopkins model offers necessary guidelines to ensure cohesion and effective changes in an organization.

Application of each Stage of Model in Proposed Implementation

Each of the stages of the John Hopkins model is essential to the proposed nursing training intervention to reduce CAUTIs. The first stage entail formulation of practice question about the health need and developing a team to assist or facilitate the implementation of the transition. It entails briefing the tem concerning the healthcare need and convincing it that there is need to solve the current problem, the rise in CAUTIs by 25% in the facility (Whalen et al., 2022). The second stage would entail imploring the team to search for and appraise evidence from different sources, especially research journals and books from the library database to identify information and evidence supporting the intervention. The next aspect would entail analyzing the collected information, and apply it to the evidence-based framework to execute the initiative.

The third stage would involve creating an implementation plan by assigning roles to different players, especially change facilitators. Next would be to evaluate the progress of the implementation based on the set timelines (Dang et al., 2021). Successful implementation would lead to sharing of the findings with different stakeholders in the facility to reduce the rates of CAUTIs.

Conceptual Map Illustrating John Hopkins EBP Model

The conceptual map for the John Hopkins EBP model shows the importance of working through inter-professional teams. Therefore, the implementation will entail having effective collaboration among the different players in the facility and not just nurses and their managers in implementing the training of nurses to reduce CAUTIs.

Phase 1: Development and refinement of the practice question and creating a team to facilitate the implementation.

Phase 2: Searching, analyzing, and synthesizing evidence from reliable sources.

Phase 3: Creation and implementation of the initiative through assigning roles and evaluating change outcomes.

Change implementation through an EBP approach requires one to deploy the most appropriate model for their project. In this case, the John Hopkins EBP model is the most suitable since it is simple and uses a systematic approach in change implementation. Further, the model entails three steps that are easy to manage and help attain better outcomes.
