The nurse professional is a senior PMHNP with an experience of over twenty years of offering care to psychiatric patients and is currently the head of the department. The nurse is also tasked with ensuring that the standards and quality of care are at acceptable standards in the facility. An interview was conducted in a bid to explore safety and care challenges at the facility. During the interview, a host of issues at the organization were explored, such as the increasing cases of hospital readmissions and poorer working conditions, which lead to higher nurse turnover. The nurse noted that the organization has, in the past, embarked on efforts and actions such as mentorship programs and educating patients before discharge. However, such efforts have only led to minimal improvement. The senior nurse professional also indicated that she has an experience with collaboration as they, at one time, worked with pharmacists and physicians to help bring down the rates of medication errors at the facility, which indicates that the organization’s culture supports collaboration.
Issue Identification
The major issue which came up during the interview which needs to be addressed is the increasing number of readmissions in the psychiatric units. Hospital readmissions have been associated with various problems, impacting the patients, their families, and the hospitals. For example, the patients and their families have to spend more money, and the patients are exposed to various hospital-acquired infections upon longer stay in hospitals (Han et al.,2020). Even though this issue is a common problem, it can be reduced or solved using various nursing interventions. The senior nurse indicated that the facility is experiencing readmission rates of up to 25%, which is considerably high. Various factors lead to enhanced cases of readmissions, such as failure to adhere to the recommended lifestyle, not adhering to the medication regimen, and relapses. An interdisciplinary approach is appropriate for this issue since the problems come from various points of care, from when the patient is admitted to the hospital to discharge and follow-up (van der Does et al.,2020). Therefore, the healthcare professionals responsible for care in these phases can all participate in the proposed care plan to help reduce the rates of readmissions.
Change Theories That Could Lead to an Interdisciplinary Solution
Change theories play a critical role in nursing as they provide frameworks upon which proposed nursing solutions can be built or implemented. The theories can also support interprofessional collaboration in resolving identified clinical problems or issues. Besides, using these theories can also support interdisciplinary solutions as professionals in the care environment can be supported to work together and find a sustainable solution to an identified problem. The proposed change theories to help in developing interdisciplinary solutions to the identified issue include the Plan-Do-Study-Act (PDSA) and root cause analysis models. The PDSA model, as the name suggests, has four different phases of change management, accomplished in a cyclic manner. The first phase, plan, entails coming up with a plan to follow. The next phase involves doing, while the third phase is studying (Li et al.,2022). The last step, arguably the most important one, entails putting every plan into action and implementing the proposed solution. The professionals in the care environment will be required to actively participate in every phase to ensure that the solution is appropriately and fully developed. This model offers a chance to restart the process depending on the outcomes of the implemented solution.
As earlier indicated, root cause analysis can also be used to help build an interprofessional solution. This model helps lead the team to find the reasons behind the identified problem and come up with plans to address the causes and not just the impact (Okes, 2019). The root cause analysis approach is also appropriate since, by applying it, the interprofessional team can work together and identify the real cause(s) of hospital readmissions and formulate a befitting solution for the problem. It is important to note the credibility and relevance of the sources cited. The sources are credible since they are peer-reviewed, and they are also relevant since they directly address the two proposed change theories.
Leadership Strategies That Could Lead to an Interdisciplinary Solution
Leaders use various leadership styles to trigger success in organizations. Therefore, the success of initiatives undertaken by the leaders depends on the leadership styles and strategies employed. The problem of hospital readmissions can be solved through an interdisciplinary approach. Hence effective leadership strategies should be used by the leaders. Among the leadership strategies that can lead to interdisciplinary solutions is the use of transformational leadership. Transformational leadership strategy has been shown to support collaboration among the collaborating interprofessional teams hence supporting the interdisciplinary solution (Iqbal et al.,2020).
As highlighted earlier, the hospital has been prompting its staff to give patients some education during discharge. Unfortunately, little success has been achieved. Therefore, the solution should have a collaborative approach targeting all points of care. Therefore, by using the transformational leadership strategy, the team leader can lead professionals at the points of care to formulate a solution that cuts across all points to effectively address the problem of enhanced cases of readmissions(Iqbal et al.,2020).
Collaboration Approaches for Interdisciplinary Teams
An interdisciplinary approach can be used to effectively address the problem. The implication is that an appropriate interdisciplinary collaboration is required. Hence relevant approaches or strategies should be used. Among the approaches is the use of open and effective communication. This approach will encourage the team members to freely express their ideas and send out communication timeously to improve their chances of success. The communication approach can be fostered through transparency, holding round table meetings, and formulating and maintaining open communication channels (Tang et al.,2018). The next approach is teamwork. Teamwork will be key in ensuring that the individuals have a common purpose, goal and pull in the same direction. Positive management and leadership will be applied to offer appropriate direction and vision for the team
Conclusion
The interview with the senior nurse offered a great insight into the problems that impact the facility. The most pronounced problem is the enhanced rates of hospital readmissions, especially in psychiatric units. Therefore, the PDSA model has been proposed to be a major model to be used in the development of an interdisciplinary solution. In addition, appropriate leadership strategies and collaborative approaches for the solution have been explored.
