Interdisciplinary Plan Proposal
The interview conducted with senior nurse professional revealed various issues within the facility, with the major issue identified as high hospital readmission rates, especially in the psychiatric department (Berry et al.,2018). Therefore, an interprofessional collaboration-based solution was proposed and therefore should be implemented at the psychiatric wing of the hospital. The focus of implementing the proposed solution is to help lower the readmission rates within eight weeks to help improve patient outcomes. As such, the purpose of this paper is to explore the interprofessional collaboration strategies, leadership and change strategies, as well as the organizational resources that will be used to help solve the identified problem.

Objective
The aim of the proposed interdisciplinary solution and plan is to identify patients who are more likely to face readmission at early states and use effective teaching strategies such as a teach-back method to the patients and their families during the discharge period to help reduce the readmission rates by atleast half within the first four weeks of discharge. The education effort entails aspects such as the importance of medication regimen adherence, strategies for preventing relapse, and lifestyle strategies to help improve care outcomes. The reduction of the rates of readmission is expected to lead to better life quality and improved health outcomes, hence ensuring that the patients can effectively fulfill their social and occupational roles.

Questions and Predictions
1. Which strategies will the team members apply in determining the specific education and learning needs of the patients and their family members? The team is expected to perform an assessment regarding the patient and their families’ readiness to learn, their learning styles, and their learning needs (Bhattad& Pacifico, 2022). The team will also perform a comprehensive assessment to determine the level of literacy and how easily they can comprehend the taught content, both written and spoken
2. How will the interdisciplinary teamcollaborate to deliver the proposed health and discharge education to the patients and their family members? The interdisciplinary team will write down various details regarding the patient, such as understanding level, health knowledge, health beliefs, and other activities(Bhattad& Pacifico, 2022). Such information will then be used to jointly formulate patient-centered and specific education strategies and health messages that the patients can understand and apply
3. How will we determine if the offered education has led to reduced readmission rates as expected? The hospital data will be used to determine the effectiveness of the intervention. For example, the team will acquire data for the rates of readmissions before the intervention and after the intervention to determine if, indeed, there are any changes regarding readmission rates.

Change Theories and Leadership Strategies
Change theories play a critical role in nursing as they provide frameworks upon which proposed nursing solutions can be built or implemented. The theories can also support interprofessional collaboration in resolving identified clinical problems or issues. Besides, using these theories can also support interdisciplinary solutions as professionals in the care environment can be supported to work together and find a sustainable solution to an identified problem. The proposed change theory to help in developing interdisciplinary solutions to the identified issue includes the Plan-Do-Study-Act (PDSA). The PDSA model, as the name suggests, has four different phases of change management, accomplished in a cyclic manner. The first phase, plan, will entail coming up with a plan to follow. The next phase involves doing, while the third phase is studying (Li et al., 2022). The last step, arguably the most important one, entails putting every plan into action and implementing the proposed solution.
The professionals in the care environment will be required to actively participate in every phase to ensure that the solution is appropriately and fully developed. This model offers a chance to restart the process depending on the outcomes of the implemented solution. The chosen leadership strategy is the transformational leadership style since it has been shown to foster effective collaboration among interdisciplinary team members when implementing initiatives and change (Pearson, 2020). This leadership style will help the collaborating professionals to come up with an interdisciplinary solution that can help reduce the rates of readmissions, especially in the psychiatric department.
Team Collaboration Strategy
As discussed earlier, an interdisciplinary approach can be used to effectively address the problem. The implication is that an appropriate interdisciplinary collaboration is required. Hence relevant approaches or strategies should be used. Among the approaches is the use of open and effective communication. This approach will encourage the team members to freely express their ideas and send out communication timeously to improve their chances of success. The communication approach can be fostered through transparency, holding round table meetings, and formulating and maintaining open communication channels (Tang et al.,2018). The next approach is teamwork. Teamwork will be key in ensuring that the individuals have a common purpose, goal and pull in the same direction. Positive management and leadership will be applied to offer appropriate direction and vision for the team.
Required Organizational Resources
The successful implementation of the proposed solution will require that various organizational resources are used. These resources will play a key role in ensuring that the educational content is well-received and understood by the patients and their family members. Therefore, the required resources include videos, podcasts, PowerPoint presentations, charts, posters, and educational brochures. In addition, it is expected that various resources will also be obtained from the organization to be used in recording videos and podcasts and delivering health and discharge education. They include internet resources, video cameras, recorders, and computers. These resources are not expected to have a financial impact on the organization since the organization already has them.
Conclusion
It is important to use appropriate strategies and identify the issues which impact healthcare organizations. Therefore, the interview revealed enhanced rates of readmission at the facility. As such, this write-up has explored an interdisciplinary proposal plan. It is expected that the proposed solution will lead to reduced readmission rates.

For this assessment you will create a 2-4 page plan proposal for an interprofessional team to collaborate and work toward driving improvements in the organizational issue you identified in the second assessment.
The health care industry is always striving to improve patient outcomes and attain organizational goals. Nurses can play a critical role in achieving these goals; one way to encourage nurse participation in larger organizational efforts is to create a shared vision and team goals (Mulvale et al., 2016). Participation in interdisciplinary teams can also offer nurses opportunities to share their expertise and leadership skills, fostering a sense of ownership and collegiality.
You are encouraged to complete the Budgeting for Nurses activity before you develop the plan proposal. The activity consists of seven questions that will allow you the opportunity to check your knowledge of budgeting basics and as well as the value of financial resource management. The information gained from completing this formative will promote success with the Interdisciplinary Plan Proposal. Completing this activity also demonstrates your engagement in the course, requires just a few minutes of your time, and is not graded.

Professional Context
This assessment will allow you to describe a plan proposal that includes an analysis of best practices of interprofessional collaboration, change theory, leadership strategies, and organizational resources with a financial budget that can be used to solve the problem identified through the interview you conducted in the prior assessment.
Scenario
Having reviewed the information gleaned from your professional interview and identified the issue, you will determine and present an objective for an interdisciplinary intervention to address the issue.
Note: You will not be expected to implement the plan during this course. However, the plan should be evidence-based and realistic within the context of the issue and your interviewee’s organization.
Instructions
For this assessment, use the context of the organization where you conducted your interview to develop a viable plan for an interdisciplinary team to address the issue you identified. Define a specific patient or organizational outcome or objective based on the information gathered in your interview.
The goal of this assessment is to clearly lay out the improvement objective for your planned interdisciplinary intervention of the issue you identified. Additionally, be sure to further build on the leadership, change, and collaboration research you completed in the previous assessment. Look for specific, real-world ways in which those strategies and best practices could be applied to encourage buy-in for the plan or facilitate the implementation of the plan for the best possible outcome.
Using the Interdisciplinary Plan Proposal Template [DOCX] will help you stay organized and concise. As you complete each section of the template, make sure you apply APA format to in-text citations for the evidence and best practices that inform your plan, as well as the reference list at the end.
Additionally, be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so you understand what is needed for a distinguished score.
