Organizational culture entails a system of assumptions, beliefs, values and norms as well as practices that regulate conduct and behaviors of individuals within the entity. These shared beliefs and values have a considerable impact on different components of processes and procedures as well as how people react to changes Melnyk et al., 2019). The mission and vision of an organization are important components of its culture as they guide how employees behave.

Cedar Sinai Health System is a non-profit and autonomous organization committed to leadership and excellence in quality healthcare services delivery. At the core of its mission are four areas that include clinical service, research, teaching, and caring for the indigents. The organization’s mission is based on ethical and cultural precepts of the Judaic tradition that inspires devotion to the art and science of healing and the excellent care offered to patients and staff (Cedar Sinai, n.d). The organization’s mission is to improve the health status of the communities it serves while its vision is to be a trusted and respected healthcare provider in the world through advancing health and healthcare within and beyond its current location. The organization is also driven by values that include integrity, teamwork, compassion, innovation, accessibility, affordability and excellence as well as diversity, equity and inclusion.

The employees in the organization value change as captured through embracing innovation, increased access and compassion as well as increasing access to excellent healthcare. The organization works through teamwork to deliver better care to patients that it serves, and hopes to expand this beyond its current location.  The leadership structure in the organization is hierarchical but founded on Judaic tradition of being dedicated to the proposed interventions.

Readiness Tool & Level of Readiness of the Organization

The selected tool to assess the level of the organization’s readiness for change is the Organizational Readiness to Change Assessment (ORCA). The tool evaluates the strength of evidence do the proposed EBP change, the quality of contextual aspects to support the change, and organizational ability to facilitate change (Crittendon et al., 2020). The survey results show that the facility attained 4 out of 5, implying that it is ready to adopt the proposed EBP intervention to reduce CAUTIs. The results show that different players in the organization have been concerned about the rates of CAUTIs and had started creating the urgency for change through proposing innovative approaches.

The current culture will support and sustain the evidence-based practice change based on the values and the mission of the organization. Innovation is a core aspect of improving care excellence and attaining Magnet recognition, which the organization hopes to attain by reducing the current rates of infections. The Centers for Medicare and Medicaid Services (CMS) requires organizations to prevent the occurrence of hospital-acquired infections (HAIs) like CAUTIs to get reimbursement as part of value-based care. As such, the mission, values, and vision of the organization are strengths for the implementation of the EBP. Possible barriers include limited nurse knowledge on CAUTIs while support from all stakeholders is present based on the management’s advancing efforts to tackle the problem. The timing of the proposal is ripe as the organization seeks to reduce the rates of CAUTIs and is willing to deploy resources to attain these goals.

Health Care Process and Systems to Improve Quality, Safety & Cost-Effectiveness

Healthcare entities focus in reducing costs, improving quality of services provided, and patient safety. Healthcare process improvement is a critical component of a strategy to enhance care and reduce cost burden. These processes include admissions, billing, discharge, ED operations, patient referrals and medication administration as well as patient flow (Melnyk et al., 2019). Organizations can only attain these processes and improve their efficiency through EBP interventions like quality improvement collaborative (QIC) De la (Perrelle et al., 2020). The approach allows facilities to implement EBP interventions and enhance overall patient safety and care.

Strategies to Facilitate Readiness

Dearing (2018) asserts that organizations show their level of readiness when they understand the perception and level of their employees’ motivation, evaluate their resources, enhance their present capacity, and empower staff to attain value for its clients and patient population. Therefore, organizational support is a core aspect of adopting EBP intervention and this starts from the leadership and management levels.

The use of models like Trans-theoretical model of organizational change (TTM) assists guide the implementation of suggested EBP (Vax et al., 2021). Change preparation is a core aspect of this model’s first three stages. As such, having shared values, exercising practical or authentic leadership, pursuing advanced education and skill and acquiring sufficient resource are important strategies to implement EBP in this facility. The organization leadership should also inspire all staff that the project will lead to patient safety and reduce CAUTIs, offer a holistic approach to care and elevate the status of the healthcare system. The leadership should ensure that the members foster a learning culture as a learning organization.

Stakeholders and Project Team Members

The main stakeholders in this EBP proposed project will include nurses, case managers, patients, educators, quality and risk manager, clinicians and nursing administrators, and the organization’s financial manager. The medical manager and nurse managers and leaders would be part of the team together with the project manager. The nursing leaders will motivate and engage nursing staff in EBP and champion the need for changes to reduce the CAUTIs prevalence. The educator will offer lessons on the best practices to use catheters and the entire catheterization process, especially use of Foley catheters.

Information & Communication

Nurses targeted in this intervention would leverage information and communication technology system to get timely, effective and convenient alerts and content to enhance their understanding of proposed best practices. The use of databases and digital sources of evidence as well as equipment to source information will improve learning and understanding of the topic by nurses (Melnyk et al., 2019). Nurses will share information and content generates from digital platforms and while the educator will leverage these tools to offer lessons remotely where necessary to improve their knowledge and application of the proposed interventions.

Conclusion

The implementation of this project will be essential for Cedar Sinai Healthcare System because of the increased role that it plays in the community. The proposed EBP requires the organization to be ready and its culture to support the changes. The evaluation demonstrates that while it is ready, stakeholders must address different aspects that may affect overall outcomes of the intervention.
