Ineffective communication is a core concern, especially during shift change and handover among nurses (Ruhomauly et al., 2019). Giving wrong medications to patients and other mistakes or errors arise from the inability of healthcare organizations managing the sharing of patient information and their profiles with colleagues. The purpose of this paper is to propose the use of Situation, Background, Assessment and Recommendation (SBAR) tool to enhance communication among nurses in the organization and reduce the occurrence of adverse events, including hospital acquired infections, and medication administration errors.

The implementation of the proposed quality improvement initiative will involve different stakeholders to form an inter-professional collaboration. The collaboration will entail different care providers including nurses, physicians and pharmacists. Nurses will work collaboratively with physicians and pharmacists to roll out the SBAR tool to enhance communication within the nursing department. Again, nurses will enlist other professionals like informaticists to analyze data on rate of medication errors and other adverse events that may reduce the quality of care offered to patients emanating from ineffective communication. Based on this data, they will implement the SBAR communication tool to improve overall care provision, especially during medication administration time to reduce and prevent the occurrence of medication administration errors.
Cost or Budget Justification for the Quality Improvement Initiative
The implementation of this initiative will be critical in reducing the overall cost burden arising from the occurrence of adverse events like medical errors and even falls. The cost of implementing this initiative will not be higher compared to the costs incurred when one medical error occurs in the facility. The facility will require resources to train nurses on the use of the SBAR tool and evaluate its efficacy with two months of the implementation by assessing the number of adverse events, especially medical errors (Müller et al., 2018). As such, the organization may spend close to $3,000 in providing necessary training and implementing the SBAR communication protocols compared to colossal amounts related to compensation and longer stays due to complications arising from such errors.
Evaluation of the QI Initiative
Project evaluation is a critical aspect in determining the effectiveness of the intervention and for its continuation. Before its implementation, sound assessment is necessary to determine its overall effectiveness in addressing the issue at hand. In this case, the evaluation of this quality initiative will entail gathering data on different aspects like medical errors before and after the implementation to ascertain its overall efficacy (Ruhomauly et al., 2019).
