The selected EBP topic is burnout among nurses. According to Shah et al., (2021), burnout is a serious threat to healthcare and health in the United States of America. The American healthcare system continues to experience changes such as increased workloads that immense pressure on healthcare providers. Nurses are increasingly predisposed to burnout because of their extensive roles in disease prevention, health promotion, and direct patient care. Burnout among nurses threatens the safety and quality of care being given to the patients, which increase the need for organizational responsiveness in implementing EBP solutions to address it. The scholarly peer-reviewed articles by Friganović et al., (2020) and Shah et al., (2021) have been selected to examine the issue of burnout among registered nurses.
Comparing between Theory, Research, and Practice in Nursing
Theory, practice, and research in nursing differ. Clinical practice act as the source of research questions as well as knowledge that nurses utilize to generate nursing theories. Clinical practice enables nurses to identify issues of priority concern in their practice and the different interventions that must be explored to promote optimum health outcomes. Research acts as a source of knowledge and evidence in nursing. Nurses utilize research to develop evidence-based practices in the professional roles. Research also generates knowledge through theory testing and development. Theory largely influences research in nursing and improves nursing practice. The theories also provide the concepts that underpin the actions that nurses take in caring patients, their families, and communities (FAAN et al., 2023). Therefore, despite being different, theory, practice, and research are crucial in nursing.
Selected Theory
The evidence-based practice change that I would like to implement to address the problem of burnout among nurses is training them on effective strategies to prevent and manage burnout. Studies have shown that strategies such as the use of breathing and relaxation techniques, seeking professional support, strengthening interpersonal relationships, yoga, and psycho-empowerment programs are effective against burnout (de Oliveira et al., 2019). Lewin’s theory of change is the selected theory that can be used to implement the above change. Kurt Lewin developed a three-stage theory that organizations can use to promote change. According to Lewin, change occurs in steps that include unfreezing, changing, and refreezing. Each of these steps is applicable to the implementation of the proposed change. For example, interventions such as creating awareness about the harms of burnout to nursing can be used in the unfreezing stage to facilitate behavioral change among nurses. Secondly, interventions such as training, coaching, supervision, and rewarding performance can be used in the change stage to enable nurses to use best strategies for burnout prevention and management (El-Sayed, 2023). In the last stage, refreezing, strategies to ensure sustained use of the best practices are implemented to prevent relapse of burnout among nurses.
Comparison of Quantitative and Qualitative Research Articles
The research articles by Shah et al., (2021) is a quantitative study that investigated the prevalence of and factors that are associated with nurse burnout in the United States of America. The researchers adopted a cross-sectional study design where data from 50273 nurses in the USA was used. The quantitative analysis of data showed that 31.5% of nurses who reported leaving their jobs in 2017 cited burnout as the reason for their intentions. Most nurses in the Southeast (30.0%) reported the increased likelihood of leaving their jobs as compared to those in the West (16.6%). The identified factors that increased the risk of nurses leaving their jobs included high workload and working more than 40 hours a week.
The study by Friganović et al., (2020) is a cross-sectional multicenter qualitative study that explored the attitudes and burnout knowledge about burnout among intensive care nurses. The researchers adopted a qualitative exploratory phenomenological design in their investigation. Nurses were chosen randomly from five hospitals based on their scores on the Maslach Burnout Inventory Scale. Semi-structured interviews were used for data collection. The data was audiotaped, transcribed verbatim, and analyzed to identify themes. The results led to themes that included stressful work, compromised private life, stress reduction options, workplace measures, protective workplace, and sense of knowledge that represented nurses’ attitudes, experience and knowledge on burnout.
Technology, Database, Search Terms
Online databases were used to identify the above peer-reviewed scholarly resources. A computer was used to search for the articles from databases that included EMBASE, CINAHL, and Google Scholar. The search terms included burnout, burnout in nursing, effect of burnout in nursing, and burnout prevention and management strategies. The search was limited to articles published over the last five years and authored in the English language.
Philosophical, Theoretical, and Methodological Perspectives
Shah et al., (2021) identify from their study burnout is a considerable threat to health and healthcare in the USA. They note that studies have continually identified the individual, organizational, and patient factors that may contribute to burnout among healthcare providers. However, few studies have examined the estimates of nurse burnout and the factors contributing to it, hence, the focus of their investigation. The authors did not adopt any theory in their investigation. However, they relied on the Strengthening the Reporting of Observational Studies in Epidemiology guideline to undertake their investigation. The methodological approaches adopted by the authors aimed at providing a true picture of the burden and rate of burnout among nurses. The use of statistics in the analysis tested the hypothesis on the influence of different factors on burnout and its estimates among nurses.
Friganović et al., (2020) assert the need for understanding nurses’ experiences with burnout to inform the appropriateness of the adopted interventions in healthcare organizations. Accordingly, their research shows understanding the experiences of nurses and attitudes would guide the implementation of nurse-centered interventions to prevent and manage burnout. The authors did not adopt any theory to guide their investigation. However, the use of methods such as inductive thematic analysis and phenomenological design enhanced the depth of meaning of the experiences and understanding of nurses about burnout.
Ethical and Scientific Integrity Issues
Research is associated with ethical and scientific integrity issues. One of them is informed consent. The participants should participate voluntarily in scientific research. They should also be able to decide to withdraw from the study any time they wish, hence, their autonomy. The other one is beneficence and non-maleficence. The intentions of the researchers should be to promote the overall good of the study participants. The study interventions should also not expose the participants to any form of harm. The data of the participants should also be keep confidential and private (McCradden et al., 2022). Unauthorized people should not be allowed to access the data. Researchers should also obtain informed consent from the participants before sharing the data with other parties.
Impact of Research on Nursing Knowledge
The utilization of research advanced my nursing knowledge. I learned more about the ways in which nurses can translate evidence into their practice. The translation supports the use of best practices to enhance outcomes in the clinical practice by nurses. The utilization of research also advanced my knowledge on facilitating change when implementing EBP. Barriers such as resistance among the users of EBP may hinder the use of best practices in nursing. As a result, the utilization of research has expanded my understanding of the different strategies that can be used to facilitate change in nursing (Dall’Ora et al., 2020). Lastly, utilization of research expanded my understanding of the different study designs and appropriateness for my future research areas. I understood better the situations where I can apply both qualitative and quantitative studies.
Conclusion
EBP is important in nursing practice. The selected EBP for focus is burnout among nurses. Lewin’s theory of change aligns with the EBP problem. Theory, practice, and research differ in nursing. Ethical and scientific integrity issues affect nursing research. Utilization of research advanced my knowledge in research and EBP.
