The patient has nutritional needs. Since he is not tolerating feeds and he is complaining of nausea and vomiting he should be given medication to reduce vomiting and the nausea and offered feeds that he can tolerate. He has emotional needs that should be carted for. Given the fact that he has chronic illnesses that have probably reduced his quality of life he should be counselled on his condition, given education on his prognosis and how to take care of himself. Patient should be made comfortable like making sure that their oxygen delivering machine is functional, prop him up in bed to improve his breathing since he has difficulty in breathing and help him in moving around.

Treatment Regimen
To manage the vomiting and nausea, give ondansetron 0.15mg/kg IV, give antihypertensive. Give insulin infusions or subcutaneous injections to achieve hyperglycemic control. For the hyperlipidemia give oral atorvastatin 10- 15mg once per day to reduce the cholesterol levels. For the blood pressure control give ACE inhibitors as they have been proven to have benefits in hypertensive patients apart from lowering the blood pressure(Messerli et al., 2018). ACE inhibitors reduce the risk of myocardial infarction. He will also benefit from low dose aspirin i.e. 75mg once daily P.O to reduce cardiovascular events(Veronese et al., 2020).
Patient Education Strategy
Patient should be educated on the importance of healthy eating and physical activities to reduce on his weight as he has a BMI of 29.8kg/m2 (overweight). This will be of great help in the management of hypertension, diabetes and hyperlipidemia should also be taught on the importance of adhering to medication since the condition he has are chronic and cannot be cured but rather controlled(Parra et al., 2021). Mr. HH should be educated against practices that will worsen his conditions such smoking and some of the danger signs to look out for while at home.

HH is a 68 yo M who has been admitted to the medical ward with community-acquired pneumonia for the past 3 days. His PMH is significant for COPD, HTN, hyperlipidemia, and diabetes. He remains on empiric antibiotics, which include ceftriaxone 1 g IV qday (day 3) and azithromycin 500 mg IV qday (day 3). Since admission, his clinical status has improved, with decreased oxygen requirements. He is not tolerating a diet at this time with complaints of nausea and vomiting.
