Reflection prompt #1: How did the case study influence your thinking about the role of the provider in preconception genetic carrier screening?

The case study elucidates that the primary responsibility of the healthcare provider in preconception genetic carrier screening is to offer guidance to women and couples before and during pregnancy, to enhance the overall health and welfare of both couples and their offspring (Prenatal Testing Can Ease Minds or Heighten Anxieties, 2019). Genetic carrier screening is a diagnostic tool that can ascertain the probability of couples conceiving offspring who may inherit recessively inherited genetic disorders. Healthcare professionals can offer patients access to genetic screening tests that are currently accessible, along with an explanation of their significance. Additionally, they can recommend that patients consult a genetic counselor to obtain more comprehensive information and interpretation of the test results.

Reflection prompt #2: What did you think or feel about SDM before this case study? How did those thoughts or feelings change after completing the case study?

Before conducting this case study, I held the belief that the responsibility for carrier screening decision-making lies primarily with the healthcare practitioner, with parents having limited involvement and primarily serving as consent-givers (Van Steijvoort et al., 2020). As a result, the majority of patients expressed dissatisfaction due to the high cost associated with genetic testing and the resulting anxiety experienced while awaiting preliminary or confirmatory test outcomes. Upon careful examination of the audio, it became evident that healthcare providers possess the ability to suggest carrier screening as a result of familial background, thereby alleviating parental distress.

Reflection prompt #3: What are the skills you will need to engage confidently in effective SDM?

The ability to actively participate in successful shared decision-making necessitates the integration of both practical experience and proficient skills. These skills encompass pattern recognition, critical thinking, evidence-based methodologies, collaborative work, and effective communication (Van Steijvoort et al., 2020). I will prioritize the development of my communication skills, as it is crucial to effectively persuade parents about the necessity of carrier screening and assist them in managing the resulting anxiety. By engaging in active listening, I will acquire a comprehensive understanding of the patient’s familial background regarding genetic disorders. This will enable me to offer appropriate and precise guidance on carrier screening, thereby facilitating informed and collaborative decision-making.

Reflection prompt #4: How will you overcome the pressures of fast-paced clinical settings to engage in SDM?

To address the challenges posed by the rapid pace of clinical environments and facilitate shared decision-making, it is crucial to integrate technological solutions, such as telehealth, to ensure sufficient time for comprehensive discussions with patients, a thorough evaluation of risks, and the provision of accurate carrier screening guidance (Lent & Brown, 2020). As a result, the act of prioritizing the risks associated with each patient can aid in the allocation of time in a manner that facilitates comprehensive and well-informed decision-making. Allowing patients sufficient time to articulate their thoughts and concerns contributes to the development of a more robust argument, thereby fostering the cultivation of critical thinking skills and facilitating the process of making informed and rational decisions.
