For the hyperlipidemia give oral atorvastatin 10- 15mg once per day to reduce the cholesterol levels. For the blood pressure control give ACE inhibitors as they have been proven to have benefits in hypertensive patients apart from lowering the blood pressure(Messerli et al., 2018). ACE inhibitors reduce the risk of myocardial infarction. He will also benefit from low dose aspirin i.e. 75mg once daily P.O to reduce cardiovascular events(Veronese et al., 2020).

Patient Education Strategy

            Patient should be educated on the importance of healthy eating and physical activities to reduce on his weight as he has a BMI of 29.8kg/m2 (overweight). This will be of great help in the management of hypertension, diabetes and hyperlipidemia should also be taught on the importance of adhering to medication since the condition he has are chronic and cannot be cured but rather controlled(Parra et al., 2021). Mr. HH should be educated against practices that will worsen his conditions such smoking and some of the danger signs to look out for while at home.
