Case Conceptualization

The case depicts Jared, a 37-year-old African American man referred to counseling by his girlfriend. Jared served four years in Operation Iraqi Freedom as a marine, and upon returning home, he was served with divorce papers. He has struggled with his divorce and homelessness in the 12 years since returning from his deployment. He has two children, 13 and 15 years old.

Presenting Problem

The patient presents with clinical features of Posttraumatic stress disorder (PTSD). PTSD can be associated with exposure to and witnessing trauma when serving as a marine in Operation Iraqi Freedom. He presents with features of PTSD, including intrusion symptoms, as evidenced by having unwanted horrific memories and flashbacks from combat (Miao et al., 2018). He also has avoidance as he tries to block the horrific memories from combat. Besides, Jared has negative alterations in mood, seen with a short temper, isolation, and being emotional. Alterations in arousal and reactivity are also present, seen with irritability and concentration difficulties (Schrader & Ross, 2021). The PTSD symptoms have created distress and functional impairment in the patient.

Counseling Intervention

Prolonged Exposure Therapy will be investigated for use with Jared. Foa et al. (2018) explain that Prolonged exposure therapy for PTSD is a manualized, trauma-focused cognitive behavioral therapy with empirical support in civilians and veterans. It entails repeated and prolonged recounting of the traumatic experience. Prolonged exposure therapy is suitable for this client since its goal is to increase the emotional processing of the traumatic event so that memories or situations no longer lead to anxious arousal to trauma or escape and avoidance behaviors (Beidel et al., 2019).
