Self-Assessment Form

The clinical practice of a psychiatric-mental health nurse practitioner is concentrated on people of all ages who are at risk of developing psychiatric illnesses or mental health issues, as well as on their families and communities (Aggarwal et al., 2022). Understanding one’s past, expertise, and experience before beginning any professional or academic endeavor is critical. Self-reflection is crucial for the development of clinical abilities, such as those required for advanced practice nursing. The purpose of this paper is to evaluate my degree of comfort doing the skills listed on the Clinical Skills Self-Assessment Form, list my strengths and areas for growth, and develop quantifiable goals and objectives for this practicum.

Summary of strengths:

Over the past few years, my experience as a nurse practitioner has enabled me to acquire a significant amount of knowledge and skills that are essential for enhancing psychiatric evaluations and treating mental illnesses. As a result of my previous professional experience, I possess a high level of confidence in my ability to conduct clinical evaluations, make diagnoses, and administer treatments. Based on the positive feedback from patients, it can be inferred that my primary area of expertise lies in oral communication. The assessment, diagnosis, and management of individuals with mental illness are heavily reliant on the patient’s medical history (Aggarwal et al., 2022). Through the implementation of proficient communication strategies such as active listening and sustained eye contact, I have successfully established a more robust rapport with my patients, thereby instilling in them the assurance to openly discuss their symptoms and experiences. Demonstrating empathy is a crucial aspect of providing care for individuals with mental health conditions, and I can exhibit such sensitivity toward their needs.

Opportunities for growth:

I have mostly treated adults, therefore I find it challenging to evaluate and treat children and adolescents, particularly when giving medicine. Certain mental disorders exhibit distinct variations in their manifestation and progression across children and adults, thereby necessitating tailored treatment approaches. Despite the significance of mental illnesses in this susceptible demographic, there exists a dearth of research on the efficacy of pharmacological interventions in treating the majority of these conditions (Kverno & Frosch, 2022). Consequently, there is a need for me to acquire further knowledge regarding the diagnosis, treatment, and assessment of mental disorders in the pediatric and adolescent populations. Therefore, it is imperative to enhance my documentation proficiency to recommend individuals for psychiatric consultation, which involves utilizing suitable screening tools and assessing the outcomes. My objective is to acquire knowledge and enhance my professional skills in providing mental health services to both children and adults with mental illnesses. I aim to achieve this by seeking guidance and supervision in areas where I have identified deficiencies during my practicum experience.

Now, write three to four (3–4) possible goals and objectives for this practicum experience. Ensure that they follow the SMART Strategy, as described in the Learning Resources.

1.      Goal: To be able to conduct psychiatric interviews on at least 5 children and 5 adults without supervision every week over the following 8 weeks (International Council of Nurses, 2020).

a.       Objective: Maintaining attention to the relevant information. Reading and practicing what I’m studying will help me learn and master the psychiatric interview, with the guidance of my preceptor.

b.      Objective: During interviews, it is helpful to ask a mix of open- and closed-ended questions.

c.       Objective: Have my preceptor do a daily assessment to determine how much time I spend with each patient and how I can make it better.

2.      Goal: Be able to correctly and independently record recommendations for consultations with psychiatrists for at least two different patients each week over the next eight weeks (Aggarwal et al., 2022).

a.      Objective: Keep a notebook close at hand for vital information.

b.      Objective: By examining psychiatric interview books from learning materials and preceptor assessments, keep track of the length of each session and develop techniques to shorten the time spent on questions.

c.       Objective: Review the documentation in the learning materials to help me keep on track with the daily documentation requirements.

3.      Goal: Find at least 5 children and teens each week for the next eight weeks, diagnose them, and treat them without supervision, using academic resources and clinical practice guidelines (International Council of Nurses, 2020).

a.       Objective: Determine what resources are accessible to families in their areas and, if required, which ones should be directed to.

b.      Objective: Involve the child or adolescent in the care plan. Take into account their preferences and, if suitable, try to incorporate them into the strategy.

c.       Objective: If I have any questions about the resources my preceptor utilizes for these groups, I will ask her for help.

4.      Goal: To compile detailed patient histories for at least five adolescents and teens who present each week for the next eight weeks with mental symptoms without supervision (Shain et al., 2018).

a.       Objective: Follow the directions in the learning resource books and the rules for interviewing children and adolescents for mental health issues.

b.      Objective: Analyze the intake data to prepare for subsequent visits. Ask the proper questions of the kid or adolescent and their parents or caregivers by the information from the intake.

c.       Objective: Obtain insightful input from my preceptor on a daily or weekly basis on how effectively I take a medical history of these groups.

Conclusion

It is clear to me from the filled-out evaluation form that I have a high degree of skill in doing clinical examinations, making diagnoses, and providing treatments. Based on excellent patient feedback, it is clear that my core area of skill is oral communication. However, there is a need for me to obtain greater information about the diagnosis, treatment, and evaluation of mental problems in the pediatric and adolescent populations to fulfill my responsibilities. As a result, I was able to develop the four goals and objectives listed above to strengthen my areas of weakness and showcase my experience in promoting high-quality mental healthcare services for all ages.

Clinical Skills Self-Assessment

Before embarking on any professional or academic activity, it is important to understand the background, knowledge, and experience you bring to it. You might ask yourself, “What do I already know? What do I need to know? And what do I want to know?” This critical self-reflection is especially important for developing clinical skills such as those for advanced practice nursing.

The PMHNP Clinical Skills List and PMHNP Clinical Skills Self-Assessment Form provided in the Learning Resources can be used to celebrate your progress throughout your practicum and identify skills gaps. The skills list covers all necessary skills you should demonstrate during your practicum experiences.

For this Assignment (just as you did in PRAC 6645), you assess where you are now in your clinical skill development and make plans for this practicum. Specifically, you will identify strengths and opportunities for improvement regarding the required practicum skills. In this practicum experience, when developing your goals and objectives, be sure to keep assessment and diagnostic reasoning in mind. As you complete your self-assessment this week, you may wish to look back over your self-assessments from prior practicums to reflect on your growth.
