The agonist-to-antagonist spectrum has two ends, one with a full agonist and the other with a full inverse agonist and an antagonist in the middle. The full agonist activates receptors to produce the desired response. The full inverse agonists stabilize the receptor in its inactive conformation and act similarly to competitive antagonists that prevent the binding of the agonist to the receptor (Berg & Clarke, 2018). The antagonist in the middle opposes the full and inverse agonist’s effects, but it cannot alter the ion channels.

A partial agonist cannot produce the maximal response the tissue is capable of, despite binding to the same number of receptors as a full agonist when it produces a complete response. This affects the efficacy of psychopharmacologic agents since the maximum desired effect is not achieved, and it takes longer before its effect is observed. An inverse agonist binds to a receptor and produces a pharmacological response opposite to the corresponding agonist (Berg & Clarke, 2018). Consequently, they cause an effect opposite to what is expected or worsen the severity of symptoms.
2. Compare and contrast the actions of g couple proteins and ion gated channels.
G protein-coupled receptors (GPCR) are proteins located in the cell membrane that binds extracellular substances and conveys signals from these substances to an intracellular molecule called a G protein. On the other hand, ion-gated channels are transmembrane ion-channel proteins that open to allow ions like sodium, potassium, calcium, and chlorides to pass through the membrane in response to the binding of a chemical messenger such as a neurotransmitter (Duncan et al., 2020). The similarity is that neurotransmitters bind both ion-gated channels and GPCR at the synaptic cleft. Besides, both take part in the exchange of ions and molecules through the plasma membrane of the postsynaptic neuron (Duncan et al., 2020).
3. Explain how the role of epigenetics may contribute to pharmacologic action.
Epigenetics can contribute to pharmacologic action owing to their significant role in developing new pharmacological agents that control epigenetic mechanisms to counteract diseases (Kringel et al., 2021). Epigenetics regulate gene activity which is essential in maintaining normal phenotypic activity of cells. This also has a role in development and diseases such as cancer and neurodegenerative disorders like Alzheimer’s.
4. Explain how this information may impact the way you prescribe medications to patients. Include a specific example of a situation or case with a patient in which the psychiatric mental health nurse practitioner must be aware of the medication’s action.
The above information has enlightened me on agonists, antagonists, partial agonists, and inverse agonists. This will impact how I prescribe medications since I will need to identify if a drug is an antagonist or agonist and how it will bring the maximal response. Knowledge of the agonist-to-antagonist spectrum will guide me in prescribing medications that will bring the desired effect and avoid prescribing those that might oppose the actions of other drugs or bring a different effect from the desired one.
