As such, the purpose of this assignment is to outline an evaluation process for the proposed health improvement project and formulate a reflection and explore project closure.

The Evaluation of The Process
The process undertaken in developing the health improvement project has been important. It offers a chance to learn lessons regarding formulating proposals and ensuring that all the parts and aspects meet the required standards. Therefore, key lessons were learned in each stage of the project cycle, from project, initiation, planning, implementation, and evaluation (Grove & Cipher, 2019). During the initiation stage, among the lessons learned was that it is important to put together a project plan documentation to guide the whole project. It is also important to identify the most relevant and key stakeholders during this phase so that the other phases, such as planning, can successfully be accomplished.
Apart from the key lessons learned in the first phase, other important lessons were learned in the planning stage. One of the lessons is that the project team should strive to timeously identify every project requirement so that the resource allocation process can be achieved successfully. Other key lessons were also learned in the implementation and control phase. One of the lessons learned is that it is important for the project’s key stakeholders to appropriately collaborate to enhance the chances of successful implementation. As part of the evaluation and closure phases, a key lesson learned is that it is important to use the key performance indicators in determining how impactful the project has been.
Analysis of the final data from the proposed project is also important as part of the project since it helps in coming up with relevant conclusions and future recommendations (Grove & Cipher, 2019). Data analysis may need the help of relevant experts as their help may be required for accurate analysis and data review. The analysis of the final data will be accomplished through the use of suitable statistical software, which can be used to produce visually enhanced outputs which can then be used in determining the impact of the project or how effective the project has been in solving the problem. It is also important to review data for quality and appropriate analysis. The implication is that the services of qualified individuals and professionals should be enlisted to help; one of them is a statistician. A statistician would give vital insights regarding the best data analysis approaches to help achieve the goals. Another professional that can help is a nurse informaticist. Nurse informaticists can offer insight into the suitability of the obtained data in further refining the remote monitoring program to reduce the rates of readmissions.
The proposed project focused on using a remote monitoring program. Therefore, if there was an opportunity to implement it, several considerations would have to be made. One of the considerations is that I would strive to use appropriate channels to acquire the resources needed for the project from the initiation phase to the implementation phase. This consideration has been informed by the fact that adequate resources form an integral part of the project completion, hence a need to procure and secure them in time. In addition, inadequate or lack of resources can have unwanted impacts on the project, such as leading to project failure (Kerzner, 2018).
Meeting the key performance indicators is usually a major target of the project as it is a show or an indication that the project has been successful. However, it is important to note that in some cases, one or more of the key performance indicators (KPIs) may not be met (Kerzner, 2018). Such an occurrence can have possible implications. For instance, the targeted key performance indicator will be that the number of 30-day readmission rates of CHF patients will decrease by 20% by week five of the implementation of the healthcare initiative. In the event that this KPI is not met, then it implies that the rates of readmissions will remain high. Therefore, the patients would continue to have poorer health outcomes and higher financial health spending.
Evidence-based data can be applied in creating a plan focusing on improving the metrics measuring the project’s structure, process, and outcomes. For example, EBP data can be obtained from a comprehensive literature review of relevant sources, such as peer-reviewed journals, published books, government websites, and institutional guidelines(Kerzner, 2018). These sources can provide the needed EBP data and similar metrics, which can then be used to appropriately create the plan
The Reflection
From the earlier discussions, it is conceivable that participation in the health improvement proposal formulation offered a good opportunity to learn key lessons which were majorly impacted by the success and challenges. Even though the project phases were completed, various challenges were encountered. For example, aligning the relevant project activities with the formulated project goals and objectives was challenging. It is worth noting that it was simpler formulating the necessary project goals and objectives. However, it was not an easy task designing the relevant activities that could help foster the accomplishment of the formulated goals and objectives.
It was also a challenge to identify the relevant and right stakeholders that would be impacted by the project and the project team that would be tasked with completing various project tasks and activities. Therefore, it was key to rely on the guidance and pieces of advice of the mentor as well as a substantial literature study on how to identify the relevant stakeholders. Even though there were various challenges, these challenges only pushed me to study more and work harder to get the right solutions. It is important to note that the health improvement project process also had processes that were easier to achieve or accomplish. For instance, identifying the problem to be used as the basis of the project and getting the relevant literature that supports the proposed solution for the identified problem.
The success and challenges of the project proposal can be influenced by various factors. One such factor is the professional experience of the project team. The level of experience of the project team can determine whether a project succeeds or would have more challenges to overcome for it to succeed. An experienced project team can deal with the project challenges to appropriately align the project activities with the formulated goals by using their expertise to ensure that each set goal and objective has the right and matching project activity. It is also important to note that proper coordination of all the activities and project team should be a priority, as well as appropriate role definition to help avert any potential overlap of roles and disagreements between the team member. The existence of such disagreements can negatively impact the project and hinder the implementation process.
The team members are also expected to coordinate and support each other for the project’s success. For instance, team members with more experience can help guide those with less experience to accomplish various tasks connected to the implementation and completion of the project. Such steps would ensure that everyone performs their role as assigned, as a failure to perform the tasks can lead to project failure.
Project scope creep issues can also occur during the project cycle. For example, they include changes in the project focuses, failure to meet the budget, and deviations from the proposed project timeline(Althiyabi & Qureshi, 2021). As such, the issues should be resolved. One strategy is to create a backup plan in time. Such a backup plan allows the team to use an alternative route to ensure the project’s success. The project team should also prioritize communication to ensure that any potential changes that can affect the project are timeously communicated to help solve the potential problems resulting from the project scope creep issues.
The key performance indicators are an integral part of the project since they can be a focal point of what needs to be achieved in the project to regard it as a success(Kerzner, 2018). The implication is that there is always a need to track the key performance indicators throughout the project. One of the reasons is that it enables the project team to have a better insight into the project’s progress. The progress can be revealed when the team members explore if the project is in the right direction in reference to the organizational goals, project goals, and strategic goals. Tracking the key performance indicators can also help inform the project team if they should adjust or change strategies and project activities to enhance the project’s chances of success.
Project Closure
Project closure comes at the end of the project and has various activities that must be accomplished. Among them is the celebration of the achievements. It is important to organize the event and officially invite every team member to appreciate their efforts (Tereso et al.,2019). The session can also be a bonding session apart from thanking every member for their roles. As part of the project closure, the organization will be appreciated for the support it offers. Such an appreciation can be accomplished by acknowledging the organization in presentations and writing official letters that show gratitude. It is also important to have a wrap-up session. The session is important since the project team can give their views and comments regarding their experience with the project from start to finish. Besides, other aspects will also form part of the wrap-up sessions, such as lessons learned and potential improvement methods.
Conclusion
The major focus of this project was to decrease the 30-day readmission rates of CHF patients by 30% by using a remote health monitoring project. This proposed project was informed by the increasing rates of readmission among patients with chronic conditions such as CHF. Therefore, various project phases have been explored, from project initiation to implementation and project closure. This project addresses the increased rates of readmission as a way of addressing a needed improvement. As such, it is hoped that this project will lead to improved care outcomes among the chosen group of patients and help them reduce the costs they spend on healthcare.
