Therefore, an interprofessional collaboration-based solution was proposed and therefore should be implemented at the psychiatric wing of the hospital. The focus of implementing the proposed solution is to help lower the readmission rates within eight weeks to help improve patient outcomes. As such, the purpose of this paper is to explore the interprofessional collaboration strategies, leadership and change strategies, as well as the organizational resources that will be used to help solve the identified problem.

Objective
The aim of the proposed interdisciplinary solution and plan is to identify patients who are more likely to face readmission at early states and use effective teaching strategies such as a teach-back method to the patients and their families during the discharge period to help reduce the readmission rates by atleast half within the first four weeks of discharge. The education effort entails aspects such as the importance of medication regimen adherence, strategies for preventing relapse, and lifestyle strategies to help improve care outcomes. The reduction of the rates of readmission is expected to lead to better life quality and improved health outcomes, hence ensuring that the patients can effectively fulfill their social and occupational roles.

1. Which strategies will the team members apply in determining the specific education and learning needs of the patients and their family members? The team is expected to perform an assessment regarding the patient and their families’ readiness to learn, their learning styles, and their learning needs (Bhattad& Pacifico, 2022). Change theories play a critical role in nursing as they provide frameworks upon which proposed nursing solutions can be built or implemented. The theories can also support interprofessional collaboration in resolving identified clinical problems or issues. Besides, using these theories can also support interdisciplinary solutions as professionals in the care environment can be supported to work together and find a sustainable solution to an identified problem. The proposed change theory to help in developing interdisciplinary solutions to the identified issue includes the Plan-Do-Study-Act (PDSA). The PDSA model, as the name suggests, has four different phases of change management, accomplished in a cyclic manner. The first phase, plan, will entail coming up with a plan to follow. The next phase involves doing, while the third phase is studying (Li et al., 2022). The last step, arguably the most important one, entails putting every plan into action and implementing the proposed solution.
The professionals in the care environment will be required to actively participate in every phase to ensure that the solution is appropriately and fully developed. This model offers a chance to restart the process depending on the outcomes of the implemented solution. The chosen leadership strategy is the transformational leadership style since it has been shown to foster effective collaboration among interdisciplinary team members when implementing initiatives and change (Pearson, 2020). This leadership style will help the collaborating professionals to come up with an interdisciplinary solution that can help reduce the rates of readmissions, especially in the psychiatric department.

As discussed earlier, an interdisciplinary approach can be used to effectively address the problem. The implication is that an appropriate interdisciplinary collaboration is required. Hence relevant approaches or strategies should be used. Among the approaches is the use of open and effective communication. This approach will encourage the team members to freely express their ideas and send out communication timeously to improve their chances of success. The communication approach can be fostered through transparency, holding round table meetings, and formulating and maintaining open communication channels (Tang et al.,2018). The next approach is teamwork. Teamwork will be key in ensuring that the individuals have a common purpose, goal and pull in the same direction. Positive management and leadership will be applied to offer appropriate direction and vision for the team.
Required Organizational Resources
The successful implementation of the proposed solution will require that various organizational resources are used. These resources will play a key role in ensuring that the educational content is well-received and understood by the patients and their family members. Therefore, the required resources include videos, podcasts, PowerPoint presentations, charts, posters, and educational brochures. In addition, it is expected that various resources will also be obtained from the organization to be used in recording videos and podcasts and delivering health and discharge education. They include internet resources, video cameras, recorders, and computers. These resources are not expected to have a financial impact on the organization since the organization already has them.
Conclusion
It is important to use appropriate strategies and identify the issues which impact healthcare organizations. Therefore, the interview revealed enhanced rates of readmission at the facility.
