The team then appraises the evidence to ascertain that it answers the aspects of the PICOT question. The third stage is the transition which entails creating and implementing an action plan, evaluating the outcomes, and disseminating the findings to the appropriate individuals in the facility (Dang et al., 2021). The third stage entails implementing changes through enabling team members to undertake certain roles to facilitate the transformation. The John Hopkins model offers necessary guidelines to ensure cohesion and effective changes in an organization.

Application of each Stage of Model in Proposed Implementation

Each of the stages of the John Hopkins model is essential to the proposed nursing training intervention to reduce CAUTIs. The first stage entail formulation of practice question about the health need and developing a team to assist or facilitate the implementation of the transition. It entails briefing the tem concerning the healthcare need and convincing it that there is need to solve the current problem, the rise in CAUTIs by 25% in the facility (Whalen et al., 2022). The second stage would entail imploring the team to search for and appraise evidence from different sources, especially research journals and books from the library database to identify information and evidence supporting the intervention. The next aspect would entail analyzing the collected information, and apply it to the evidence-based framework to execute the initiative.

The third stage would involve creating an implementation plan by assigning roles to different players, especially change facilitators. Next would be to evaluate the progress of the implementation based on the set timelines (Dang et al., 2021). Successful implementation would lead to sharing of the findings with different stakeholders in the facility to reduce the rates of CAUTIs.

The conceptual map for the John Hopkins EBP model shows the importance of working through inter-professional teams.
